Forms and Policies

Lakeside Presbyterian Church seeks to provide a safe and secure environment for the children and youth who participate in our programs and activities. Along with our Child Protection Policy, we have a Bullying Policy, a Volunteer Code of Conduct and a Participant Code of Conduct. We often ask that parents, volunteers, and participants turn in forms.



CHILD PARTICIPATION PACKET

For each child birth through 5th Grade. To be filled out by parents and child. Includes “Registration, Release, and Signature Sheet”.



YOUTH PARTICIPATION PACKET

For each youth, rising 6th Grade-12th Grade. To be filled out by parents and youth.



ADULT PARTICIPATION PACKET

For all adults (18+) who work directly with our children or youth.


